The California State Preschool Program (CSPP) is a free program offered to families who meet the income and family size criteria provided by the State of California
 
Placement into a preschool class is based on family ranking, not first-come, first-served 
 
 
APPLICATION AND DOCUMENTS REVIEW
 
All required documents listed in both STEPS 1 & 2 MUST be submitted with the application per California State Preschool Requirements. If you are missing documents, you will be notified of the documents needed and your application will be placed on hold until we receive all the remaining paperwork.

CHILD ELIGIBILITY
 
A CHILD MUST:
  • Turn three (3) or four (4) years of age between December 2, 2023, and December 1, 2024
    • For 4-year-olds - The child's birthdate must be between December 2, 2019, and December 1, 2020
    • For 3-year-olds - The child's birthdate must be between December 2, 2020, and December 1, 2021
  • Have a negative TB skin test reading given on or after August 14, 2023, and be current with all required immunizations
  • Have a current physical examination report after their last birth date
 
FAMILY ELIGIBILITY
 
THE FAMILY MUST:
  • Be the responsible person in providing the care and welfare for the preschooler
  • Provide own transportation to and from school - No Bus is available
  • Provide all required documents
  • Qualify by income & family size
 
 
INCOME QUALIFICATION
  • Family income must be at or below the state guidelines as listed below
  • Proof of income will be required to verify the family's qualification

STATE PRESCHOOL ENROLLMENT APPLICATION PROCESS
There are two STEPS to submit your application for State Preschool
 
All required documents must be submitted in either pdf or jpg format in order to process your application
 
Step 1 Gather the following documents and submit your New Student Enrollment Interest Application by clicking on the link provided below.
 
PROOF OF AGE ELIGIBILITY - Provide proof of the child's birth record. 
 
Acceptable Proof of Birth Records
 Birth Certificate Hospital Print Out
 School or Medical Records
Child Custody Court Order
Adoption Documents  County Welfare Department Records
Foster Care Placement Records  Other reliable documentation indicating the relationship of the child to the parent
 
PROOF OF IMMUNIZATION - Provide your child's most current immunization record along with a negative Tuberculosis skin test reading or a TB Risk Assessment given on or after August 14, 2023.

PROOF OF RESIDENCE - The document MUST be current and in one of the parent's names.  See below for acceptable proof of residence documents.

Cable Bill Phone Bill
Employment Pay Stub
Lease Agreement with a current month's receipt
Mortgage Statement Property Tax Statement Other reliable documentation for a service of the home
 
If your family is living with someone:
  • Submit a copy of the homeowner's proof of residence in STEP 1 and have the homeowner complete and sign the Declaration of Residency Under Penalty of Perjury to be submitted in STEP 2
If your family currently does not have a permanent residence:
  • Complete and sign the Self-Identification of Homelessness form in STEP 2 only.  Proof of residence will not be required.

 Step 2 Complete the required forms listed below and attach the required documents in your email to our office at [email protected] with your child's first and last name in the subject line.
 
FAMILY APPLICATION - Download and complete the 2024-2025 State Preschool Family Application form below
PROOF OF FAMILY SIZE - Birth Records must be provided for ALL of the enrolling child's siblings counted in the family size and living in the home who are under 18 years old and listed on the Family Application. The same birth record documentation requirements apply as listed above for your preschool child.
 
PROOF OF RESIDENCE - Part 2 - If the Proof of Residence provided in Step 1 is in the name of the person your family is living with, then you must submit a completed Declaration of Residency Under Penalty of Perjury form signed by the homeowner.
If your family currently does not have a permanent residence, then you must submit a completed and signed Self-Identification of Homelessness form
PROOF OF INCOME - Documentation for ALL of your family's sources of income must be provided by each parent who is responsible for the care and welfare needs of the family.

Below are the required forms according to your family's source of income
PROOF OF PHYSICAL - A current physical examination report after the child's last birth date must be provided.  If you do not have a copy from the doctor, please have your child's doctor complete, sign and date the Physician's Report
  • Please note, a current physical report must be provided on a yearly basis to continue enrollment.
CHILD'S PREADMISSION HEALTH HISTORY - Download and complete this form to provide general information about your child's development and health
PARENT/GUARDIAN IDENTIFICATION - Provide a copy of each parent/guardian Driver's License, ID, or Passport that is listed on the Family Application.
 
OTHER REQUIRED DOCUMENTS
  • IEP Document - Provide a copy if your child has an Individualized Educational Program (IEP)
  • Court Documents - Provide a complete copy of any court documents you may have about your preschool child (i.e. Child Custody, Restraining Order, Guardianship, etc.)
 
FOR CHILDREN LIVING WITH A GUARDIAN OR FOSTER PARENTS ONLY
  • Foster Parent’s Placement Agreement (SOC 156) – For Foster Children
  • Court Order Appointing Guardian of Minor – For Guardianship Children
  • Caregiver Affidavit - If the child is a Foster Child and/or in Guardianship and you do not have a Foster Parent's Agreement (SOC 156) and/or a court order Appointing Guardianship, then you must submit obtain a Caregiver Affidavit from the Student Support Services Department at the Central Unified School District Office, 5652 W. Gettysburg, Room 13.  Please call 274-4700 ext 10138 for any questions regarding the Caregiver Affidavit.
