Fee Based Preschool

CENTRAL UNIFIED FEE BASED PRESCHOOL 
is a paid program offered to families at $22.00 per day
 
Placement into Fee Based Preschool is based on a first come, first served basis
 
2020 - 2021 QUALIFICATIONS FOR FEE BASED PRESCHOOL
 
A CHILD MUST:
  • Turn four (4) years of age between December 3, 2019 and December 1, 2020 of the school year from August 12, 2020 to June 10, 2021
    • Your child's birth date must be between December 3, 2015 and December 1, 2016
    • Please note:  Children who turn three (3) years old by September 1, 2020 may also be eligible for preschool
  • Be completely toilet trained and be able to use the restroom without any adult assistance
  • Have a TB skin test negative reading given on or after August 12, 2019 and be current with all immunizations
  • Have a current physical examination report after the last birth date
THE FAMILY MUST:
  • Live within Central Unified School District
  • Be the responsible person in providing the care and welfare for the preschooler
  • Provide own transportation to and from school
CLASSES
Morning classes are available at Teague Elementary School.  
 
Parents may choose classes from 2, 3, or 5 days per week depending on their preference and class availability.

FEES
Cost for classes are $22.00 per day.

FEE BASED PRESCHOOL ENROLLMENT APPLICATION PROCESS
There are TWO STEPS to submit your application for State Preschool
 
All required documents must be submitted in either pdf or jpg format in order to process your application
 
STEP 1 Gather the following documents and submit your New Student Enrollment Interest Application on the link below.
 
PROOF OF AGE ELIGIBILITY - Provide proof of child's birth record. 
 
Acceptable Proof of Birth Records
 Birth Certificate Hospital Print Out
 School or Medical Records
Child Custody Court Order
Adoption Documents  County Welfare Department   Records
Foster Care Placement Records  Other reliable documentation indicating the relationship of the child to the parent
 
PROOF OF IMMUNIZATION - Provide your child's immunization record along with a Tuberculosis skin test negative reading given on or after August 12, 2019.
PROOF OF RESIDENCE - The document MUST be in one of the parent's names and dated within 30 days. 
 
If the family is living with someone, a signed Declaration of Residency Under Penalty of Perjury must be submitted in STEP 2.

 STEP 2 Complete the required forms listed below and submit along with your required documents to our office email preschooldocs@centralusd.k12.ca.us with your child's name in the subject line.
 
ENROLLMENT APPLICATION - Download and complete the 2020-2021 CUSD Fee Based Preschool Enrollment Application form below
PROOF OF RESIDENCE Part 2 - If the Proof of Residence provided in Step 1 is in the name of the person your family is living with, then you must provide a completed and signed Declaration of Residency Under Penalty of Perjury.
PROOF OF PHYSICAL - A current physical examination report given within the past year must be provided.  If you do not have a copy from the doctor, please have your child's doctor complete the Physician's Report
  • Please note, a current physical report must be provided on a yearly basis to continue enrollment.
CHILD'S PREADMISSION HEALTH HISTORY - This form must be completed by the parent and provides general information of your child's development and health.
OTHER REQUIRED DOCUMENTS
  • Driver's License/IDs - Provide a copy of each parent(s)/guardian(s) enrolling the preschool child
  • Court Documents - Provide a copy of any court documents you may have pertaining to your preschool child
  • IEP Document - Provide a copy if your child has an Individualized Educational Program (IEP)
  • Caregiver Affidavit - If the child is a Foster Child and/or in Guardianship and you do not have a Foster Parent's Agreement (SOC 156) and/or a court order Appointing Guardianship, then you must submit obtain a Caregiver Affidavit from the Student Support Services Department at the Central Unified School District Office, 5652 W. Gettysburg, Room 13.  Please call 274-4700 ext 63138 for any questions regarding the Caregiver Affidavit.
 
 
NEXT STEP                 APPLICATION AND DOCUMENTS REVIEW
 
All required documents listed in both STEPS 1 & 2 MUST be submitted with the application per Fee Based Preschool Requirements. If you are missing documents, you will be notified of the documents needed and your application will be placed on hold.
 
Once all documents are received. You will be notified of the status of your application within 30 days.  If we are able to place your child into a class, we will contact you to schedule an appointment to complete the registration process.
