26-27 Enrollment Requirements
 
  • DOCUMENT REQUIREMENTS

  • Proof of Birth (Birth Certificate or Baptismal Certificate)

  • Proof of Residence (Utility service bill, mortgage statement, rental property contract & receipt, property tax payment receipts, pay stubs, voter registration, and declarations of residency)

  • Proof of a California Tuberculosis Risk Assessment signed and completed by a licensed physician or Central Unified Health Services

  • Proof of Immunizations must be up-to-date (See Below)

    • Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or TD) - 5 doses  (4 doses OK if one was given on or after 4th birthday)
    • Polio (OPV or IPV) - 4 doses  (3 doses OK if one was given on or after 4th birthday) 
    • Hepatitis B - 3 doses
    • Measles, Mumps, and Rubella (MMR) - 2 doses(both given on or after the 1st birthday)
    • Varicella (Chickenpox) - 2 doses
